Think my piggy may be dying. How do I know?

No! She’s drinking less than usual

OK - so diabetes or kidney disease is rather unlikely. Either issue is often characterised by increased drinking and heavy weight loss but not necessarily much in the way of other symptoms.
 
They’ve put her back on anti inflammatory medicine/ pain killer for three weeks to see if that helps and then i have to take her back and if she hasn’t gained any weight they’re gonna do a blood test.
I suppose if she's been trucking on for this long they want to have a good go at seeing whether controlling pain and/or inflammation makes any difference - i.e. if she starts to eat more and put on weight maybe a cancer is less likely? If you'll be waiting 3 weeks he must think it's a long-term situation rather than something acute and urgent - it looks like they are treating symptoms first then looking for a possible cause later. Am I right in thinking that taking a blood sample involves a general anaesthetic? Maybe that's why they want to wait and see...
