I think my guinea pig died from an overdose.

Hi everyone, this is the first time I have used The Guinea Pig forum and sadly, my guinea pig passed away a few weeks ago, but I am absolutely sure she died of an overdose after seeing the emergency vet. I would really welcome anyone's thoughts or opinions, or if sadly, you have experienced the same thing.

Bonne was prone to urine infections and so we have seen our usually vet many times, she also had an operation to remove stones which she came through well. At the first sign of squeaking when peeing, I rushed her to the vet to start treatment as I know how quickly infections can get a hold. It was the weekend and so I had to see an out of hours vet. Bonne was absolutely fine otherwise, she was running around, eating veggies and hay, standing up at her cage, coming when I called no signs of anything else other than squeaking when peeing and this had only just started. I gave her 0.1ml metacam at 12pm and we saw the vet at 2pm. The vet seemed not to know much about guinea pigs. I told her it was common for Bonne to get urine infections, I had changed her diet and her cage was cleaned x 2 daily to try to avoid getting them. I also told her this was the first squeak but just wanted to make sure she started treatment. I told her I had given her 0.1ml metacam already which helped. She examined Bonne, and said no tenderness, no lumps, and Bonne didn't even squeak when examined. She tested her temperature, and it was normal. The vet proceeded to weight her she was her normal weight. Then unlike any vet before gave her an injection of Buprenorphine 0.5ml. There was spillage and so she did it again. I could cry when I remember this, I just trusted what she was doing but thought after how could she have possibly known how much went in the first time. She said Bonne would be very sleepy but it's not the end of the world (famous last words for Bonne). From the time she had the injection she didn't move. We came home I put her back in her cage she wouldn't move, she couldn't even hold her head up. She wouldn't eat, or have water. I tried to give her very watered down critical care but she just spat it out. I offered her all the veggies she loves, she didn't even notice they were there. I just kept trying with the syringe with critical care and water. I covered her for a short time, when I went back it was like she was having a seizure, she was jolting and then passed away which broke my heart.

I would really appreciate any thoughts on this situation, thanks so much in advance.
 
I am so sorry you lost Bonnie. I could not comment on the injections but hope someone might know more about the drug and its use. Have you contacted your vets? perhaps if you wrote them a letter or emailed them for answers it may help. It may be she had a bad reaction to the drug itself.

Thank you so much for replying. Yes, I contacted them a couple of days after it happened, I couldn't bring myself to contact them before that. They asked for 35 days to reply and then extended it until I receive a reply last night. They are not the vet I would usually go to for the piggies, but out of hours there was no choice. When they replied they got my name wrong, the surgery I visited wrong they said Doncaster when I'm down in Kingston, they even got the date of the consultation wrong. They said it would make no difference if she was injected twice which I don't understand. They also said it must have been an underlying condition but I know in my heart it wasn't. They said they couldn't say definitely without a post-mortem. I have replied to them and will report this just in case there was something with the double injection as I really don't want anyone else to go through this. You do have a good point about the bad reaction to the drug itself as she had never been given this drug before, so you might well be right.

I can’t give any advice but just wanted to say it really sounds like you did absolutely everything you could in that situation, so you should try not to feel guilty as it was beyond your control.
 
I am very sorry for your loss 😞 Buprenorphine is a strong opioid painkiller. I can’t comment on the dose that Bonnie was given but when my Ella had it, she was given 0.06ml orally 3 times a day for a few weeks post surgery. I know she was also given it via injection after the surgery and it made her very spaced out and sleepy but I’m afraid I don’t know the dose.

Whether or not Bonnie died from an overdose, I do not know, hopefully someone more experienced with medication will be along shortly but if you have any concerns over it, please do speak with your vet.

Please don’t feel guilty, Bonnie knew how much she was loved x

I’m so sorry you and your little lady had to go through this. If you are not happy about that response look into taking it further to a higher level. It’s awful that they can’t even get simple facts right like your name and location 😖

Thank you so much. It was horrible as I did feel so guilty as I had taken her there but then I just had to make myself realise that it was with the very best intention to get help for her. Thank you again.

Oh how sad, what a horrible thing to go through. It's unlikely she would have known what was happening at least as the buprenorphine will have made her very sleepy.

Thank you x Yes I was so angry when I saw the response last night, they had already extended the initial 35 days to respond and when I read the reply it felt as though they hadn't taken any care in the response or investigated it properly. I wrote back immediately and said that I want my initial complaint escalated, to include their response to me yesterday. My little Bonne deserves more than this. Thanks so much for replying x
 
You and Bonne both deserve a proper explanation and apology. I hope you get it! I complained to our local emergency vet after taking my house rabbit there one night. It was a long story but in the end they sent her a box of goodies to cheer her up!
 
It does sound huge. A quick look online with my sister's help (student vet nurse) suggests a dose range of 0.01mg - 0.05mg/kg MAXIMUM.

If she was given 0.5ml, provided that's at the standard strength 0.3mg/ml and she is an averagely sized guinea pig of around 1kg she will have ended up getting 0.15mg/kg, a three times overdose!

0.5ml @ 0.3mg/ml x 1kg = 0.15mg

I'd get in touch with the vet and ask for an itemised list of everything given to her on that day, and the amount of each. It should have the amoint of the med, the name of it and how much was given.

I thought it sounded like a huge dose after what I was instructed to give Ella but wasn’t sure as I’m not experienced with medication

I’m so sorry. I’d definitely take it up with them and I’d even be inclined to complain to the RCVS if it’s true that she was given a huge amount and passed away from an overdose x
 
In human medicine you would not give another dose,if there was spilage.As you would never know how much the patient recieved.
Any morphine based drug can depress breathing,and neurological function and slow down the gut.

I agree with others it would be good to get a itemised bill.report your concerns to the RVCS.
Just because guinea pigs are small and exotic they deserve a knowledgeable vet,or willing to admit they are not sure,but will find out.
 
I agree with the others. Unfortunately nothing will bring back your special girl but you pursuing this can give you a sliver of comfort with the knowledge that you may save a future small furry. Be assured that with an opiate in her system she won't have suffered.

I am so pleased they at least sent a box of goodies for your rabbit ❤️I'm sorry you had to go through this experience, it's worrying enough having to take our pets to an emergency vet, without problems on top of that. Thanks so much for your reply.

I have doubled checked my invoice again just to be sure. I didn't actually use any of the other medications as Bonne passed away. As for Buprecare
our normal vet never prescribed this. Yes she was 1.1kg. Thanks so much again.

Thanks so much. They are escalating it as I sent another complaint about their reply, but I think you are right I will make a complaint to the RCVS as I really want this investigated I would hate to think this could happen again x
 
Last time I used buprenorphine it was given as transmucosal, I was told that the dose was much higher than what I had previously been given because it was harder for the body to absorb that way, so the fact that the injectable dose is higher than the transmucosal one should surely have been a red flag to them! Especially not to give double.. I'm so sorry for your loss. I hope you're able to get a satisfactory resolution with the vet
