Vet letter templates - is there one to leave with relatives caring for pigs whilst their owner is away?

I have searched the forums, but couldn’t find anything.

Basically my Christie is now 6 and has ovarian cancer. I, and my vet have decided not to treat but to leave her to continue until she tells us she’s had enough.

We’ve got a family commitment trip this weekend so the pigs will be going to their holiday lodging with my parents. I need to leave some sort of letter with Mum for Christie just in case it’s needed giving Mum. Any advice/templates to give her a sort of veterinary’power of attorney’ for the weekend?

I am leaving in October. I was just going to leave daily care things like hay and water on a paper. Have previously cut vegetables in a plastic bag, in the fridge with days of the week on it. Then of course, vet info. I may also use disposable bedding in their cage, since it does not have to be spot cleaned every day.

You should just make a note about what to do in case something happens.
- the vet you use
- the phone number of the vet
- what has been happening
- how much you will let them spend in the office
- any info on insurance
- of course how to get in contact with you
- what to feed her if something happens
 
I’m sorry to hear she is unwell.

I think it would only need to say something along the lines of - i I give xxxxx permission to make decisions on my behalf regarding Christie’s care.

You could also call the vet actually because they may 1) not actually need anything or 2) make a note in their records jusr for this weekend.

I know my parents have taken in my animals during our absences and the vets didn’t need anything like that so it is worth asking
 
Whenever I have gone away when I have a guinea pig on vet treatment , I have informed the vet by ringing reception . All I have needed to do is inform them that I give (I give the name of the person ) permission to bring my guinea pig for appointments and make decisions about their care
 
When my piggies go on their holiday to Wiebke’s I write a letter saying ‘as the owners of guinea pigs Elizabeth, Ella and Esme ******* I, Claire ******* give my permission for Mrs Wiebke ****** to make any decisions including euthanasia if my guinea pigs require veterinary treatment while in her care’ I then sign it

I also inform my vets that I’m going away and I give my permission for their medical history to be disclosed and my vets put a note on my file.
 
The boarder I use needs a tick (in a box) for my agreement to veterinary care and a signature. Perhaps you could just write a clear note that says you give permission for (piggies name) to get veterinary care and then give your signature.

Thanks, that’s what I need. It’ll be at a different vets to my usual practice. 🤞it won’t be needed, but wanted to be prepared just in case.
