Unknown loss of hair/sores

I have a 3 year old female guinea pig who has been having sores and patches of loss of hair on her back, belly, legs. In addition, she has had white spots on her nose and ears. They are all very itchy to her. We have been to the vet many times. At first they thought it was fungal. She had two rounds of anti-fungal oral medication. It continued. They did a skin scrapping which verified that it is not fungal. Then they thought it was mites. We had two rounds of revolution treatments for suspected mites but it has come back. The other strange thing is that she is paired with another guinea pig who hasn't had any of the symptoms. My vet is at a loss and doesn't know what else to do. Does anyone have any ideas of what else it may be? After $1000 in vet visits, tests, and treatments- I would really love more advice. I’m sorry to hear you’re struggling with your girl. That does look very sore. I don’t have a clue what it could be I’m afraid. But hopefully someone more knowledgable may be able to help you on how to move forward.

Poor thing does look sore. Not all fungal is ringworm, so I’m wondering if your vet has explored other fungal treatments? What fungal treatment did she have?
 
It does have a look of fungal about it, I don't think skin scrapes are always 100% accurate so this might still be a possibility. If it is definitely not fungal has your vet considered a microbial/bacterial skin infection? Microbial skin infections require bathing in anti microbial prescription only shampoo such as malaseb (uk)
