Skin and leg problems

I’ll get straight to the point, my guinea pig (about 4-5 years old and male) started having skin issues about a month and a half ago. It was just bald patches and a few wounds where he had been biting at it. Took him to the vet and he got a steroid cream which had worked. Few weeks later it happened again but a lot worse, it was on his back, ears, legs, stomach and he was in pain. Vet gave antibiotics and painkiller which it seemed to be getting better however now one of his front legs is red and swollen and his two back legs are swollen and he can only hop instead of walk. He’s still eating, drinking less but if I bring the bottle over he drinks a lot and pooping normally. Does anyone know what this could be? Is there anything I can do to help his legs or just let him rest?

This sounds to be a pain issue - something internal. They can bite and chew at areas due to pain.
Please do take him back to the vet so further investigations can be carried out.
Is he still on a painkiller?

He’s still on the painkiller twice a day. If I lift him up he can use his back legs normally. Front leg doesn’t have any wounds is just red and swollen. Looking at him now he seems to be hopping about more now that he’s got the painkiller so I’d say it is a pain issue. I brought it up to the vet last time he was there but the vet was more concerned about the skin rather than the legs.
