Post neuter complications/diarrhea on Baytril

Hello everyone! I am from the US and have 5 piggies - 3 neutered boars and 2 females. The most recent addition to the family, Murphy, had his neuter the first week of December by a cavy savvy vet. We have had complications ever since then. He did go home with meloxicam but she did not want him on a preventative antibiotic.

Just a couple of days after the surgery he developed an infection in both incisions. Murphy was a very naughty picker. This vet didn’t use packing or staples but rather dissolvable stitches so those may have irritated him. For the infection he was prescribed a course of sulfatrim which he tolerated very well. As soon as he started that we began Benebac which is the most accessible probiotic here. We cleaned the wounds with diluted hibiclens and slathered them in Silver sulfadiazine cream. Kept him topped off with CC. At one point his poops began to soften but the addition of cisapride fixed that almost immediately.

Fast forward to 3 days ago, Murphy finished the sulfatrim and was doing very well. Eating on his own, popcorning, and wheeling. A day later, I noticed he couldn’t fully sheath his penis. I wasn’t concerned until a few hours later that same day when he had more discomfort, swelling, and redness. We were able to take him back to our same cavy vet yesterday before the New Year weekend and she confirmed that he had a penis infection and likely a UTI as well. She insisted on prescribing Baytril/enrofloxacin as she said even though it was a big gun it was the best antibiotic for this specific infection. She did not feel the need to do a culture.

Now we are to today. My little Murphy has diarrhea after just two doses (.15ml) of Baytril. I am able to text and communicate with the vet and she says the best option right now is to keep him on it. I am syringe feeding every 2 hours and have added poop soup from one of his healthy lady friends. He is still on the cisapride, Benebac, and meloxicam (.3ml twice daily or one dose of .6ml). He is uncomfortable but not crying with defecation. Doesn’t want to eat or drink on his own.

I know the main thing is persevere and don’t give up on him. I feel so bad because I just wanted him to be able to hang out with his lady friends and he has gone through the gauntlet for weeks. My question is, am I missing anything? Should I add or take anything out? How else can I help him be more comfortable? There is a new emergency vet nearby that will occasionally have an exotic vet on staff but it’s up to luck and I’m not sure what they could do. My vet said to watch for “decline in status” and blood in the feces.
 
I am so sorry you and your piggy are going through this. I am not a health expert so can't offer you advice but hopefully one will be along soon.
 
I've not heard of cisapride being given because of soft poos, more if poos stop or become infrequent. One of my recent vet trips for bloat, where I was prescribed metoclopramide (another gut stimulant like cisapride) I was told to immediately stop if diarrhea happened. The vet also didn't prescribe me cisapride along with metaclopromide for that reason, so it may be the cisapride overstimulating the gut and causing the diarrhea.

This is just one vet's opinion though, of course.
 
Hello everyone. Quick update. Murphy passed in my arms after some small seizures. I had decided to discontinue the baytril and cisapride. Please be wary with baytril and multiple antibiotics. We were doing everything we were told and trying our hardest and he still didn’t make it.
 
I'm so sorry, I also lost a guinea pig yesterday quite suddenly so I know how you feel. It sounds like you did everything you could, and that he went quickly in the arms of someone he cares about.

Yeah it never gets easier, but at least you have the experience to know you'll be ok at some point. And thank you, all the best.
