It looks like mucus which could suggest digestive problems.

I'm sorry to hear the vet says he needs to be put to sleep.
If this hasn’t yet happened, can I ask a few questions -
How old is he?
Was his urine tested for a UTI?
X rays? - this can help identify whether it may be stones rather than a UTI
How long has been on antibiotics?
Have you been weighing him daily? Lack of poop can suggest stasis/not enough food, so the weight checks would tell you whether he is getting enough food to keep his weight stable.
When did he last poop? Poop output is 1-2 days behind food intake
How much syringe feed is he getting in each 24 hour period?
Was he given a probiotic (or poop soup made from the poops of a healthy companion) while on antibiotics? This can help settle the gut and lessen the affects of the antibiotics on the digestive system

Hi he's around 6 he was a rescue before so not to sure, he had an ultrasound about week ago wasn't any stones, but there unsure whether he had a lump or something else.
He has been on antibiotics since 16/07 he's also on meloxidyl and emeprid. He's put on weight over last few days.
I'm feeding critical care about 4/5 hours and water at the same time about 10ml a time.
He wasn't given probiotics at the time or poop soup but I have given it to him now.

Thinking of you today. I’m sorry you are having to make this difficult decision. ❤️
