In my experience this is usually due to a build up of hair, hay and/or smegma stuck around the shaft, which prevents it being able to retract fully. If you have any water based lubricant or coconut oil will do in an emergency, you should be able to gently expose his penis and clean away anything forcing it out, there's a guide on how to clean their penis in this guide: Boar Care: Bits, Bums & Baths
Using the lubricant will help it come out/go back in if needs be. If its still sticking out after being cleaned, or if there's nothing forcing it out, it would be best to have him examined by a vet in case of a prolapse.

I would gently try and put the penis back in the skin.if that's not possible it will need a vet to check it.
