Interstitial Cystitis cocktail of treatments—advice?

My two female guinea pigs have been having recurring UTI’s for over a year now. Only once has any bacteria been found in one of their urines which was the last time in December but that was treated with antibiotics. The bacteria went but the blood in the urine stayed along with squeaking when doing the toilet. Both have continued to squeak persistently even between flare ups where blood is found on the fleece (when this happens they are seen by the vet right away)

Since summer 2021 they’ve both been taking one pill of cytease cat bladder supplement mixed with water (1ml served over two 0.5ml portions morning and night) This was meant to be a preventative and worked at first but obviously the flare ups have prevailed. It’s meant to thicken their bladder walls but obviously I can’t see inside lol.

Now my vet has prescribed metacam and gabapentin for daily use to help manage inflammation and pain respectively. This means they are taking a lot of medicine and it’s getting expensive. I’m prepared to pay what I need to keep my girls comfortable and will hopefully be able to buy online as my vet says they’ll provide the prescription. My issue is the gabapentin dose is 1ml once a day. 1ml in one sitting is quite a lot and I tried to give it to them with their metacam and cytease tonight and it was too much. Their bladders must’ve struggled because they started resisting at about 8ml and I had to put them back. I have decided it will be given separately a while before their other night time medicines.

now their medicine schedule is as follows:

morning: 0.4ml metacam and 0.5ml cytease
after humans dinner (6-7:30pm ish): 1ml gabapentin
evening: same as morning

does anyone else have similar experiences with stubborn utis? I feel like a bad owner at times because they’re so uncomfortable and always at the vets. It’s horrible to see them on more medicine especially since they don’t seem to like taking the gabapentin.
 
I have a girl with IC, the best I have found to ease her symptoms is 100mg glucosamine and 100mg d mannose daily. I’ve been able to take her off meloxicam daily and only use it for bad flares. Our exotic vet is also doing pentosan/cartrophen injections weekly for 4 weeks and then going to monthly for 6 months. He said he’s had other Guinea pigs with IC do well on this and this is meant to help repair the bladder also. We’ve done two weeks and on week three now. I’m in Australia and these injections were quite affordable, you could ask to see if these would help your piggie.
 
My vet has prescribed Gabapentin in 0.3ml three times a day. It seems a lot to give 1ml in one sitting. Do you know the strength of the Gabapentin?

It says on it ‘gabapentin soln 50mg/ml (bttl of 150ml) x 60.0‘

I'm not sure if spreading it out during the day would make a difference though as their metabolism is quite fast, so maybe it just goes through quite quickly? Or maybe he was conscious they have 2 other split doses and wanted to keep it manageable.

any thoughts welcome and I can bring it up at my next check up, I’m happy with my vets overall knowledge though—they’ve been very piggy savvy and open to my questions as worried pig mum lol. I get the vibe it might be the first time they’ve treated such w stubborn pair of pigs though.

Sorry for the late reply. Spreading the dose through the day is helpful with their fast metabolism. Maybe just talk to your vet and see what they say. It just seems a lot to give at one sitting. Good luck
