Internal Cyst in male guinea pig - surgery advice

Hi,

My guinea pig Gonzalez has been ill for the past two weeks - I suspected he had an URI so the vet put him on metacam and antibiotics but then he stopped eating, lost weight, poos went mushy, wasn’t active and also started squealing when passing some poos. (Back in July he was also on meds and they did the same thing making him squeal when pooing, vets said it was just a side effect and it stopped after he came off them). He went to the vets again and they said he seemed fine but they decided to take him off the meds as he could have upset his gut, and he’s been put on critical care. He still hadn’t improved so he went in again on Monday to another vet and this time she felt a lump on his tummy and she said he squealed if she touched it. He’s back on a high dosage of metacam and since has improved, he started eating normally, poos are back to normal (still slight squeaking though when he passes some) and he seems so much happier, running around and is popcorning lots.

He had an ultrasound on Tuesday to see what the lump is and unfortunately he has 2 internal cysts (1cm diameter) close together filled with fluid on his tummy/abdominal. The vet says this is why he’s been feeling ill (nothing to do with a URI or the antibiotics) but she is very confused because she doesn’t actually know what it is or what’s caused it as he’s a healthy 1 year old male. She spoke to an exotic specialist who has said the only option forward is for them to remove it, otherwise he’ll be in pain and it might grow and burst. But he also didn’t know what it could be. (Which concerns me) . I also cannot find anything online about abdominal cysts in males.

The operation is meant to be happening next week but I am SOOO concerned about it. Gonzalez would need to be put under and they open him up and see what the lump is. If all went well during the operation they would be able to remove the cyst, stitch him back up and he wakes up fine. But obviously there are very high risks with anaesthesia and guinea pigs, especially as they are opening up his abdominal. Another thing is, because she has no idea what it is they might open him up and find they can’t do anything to the cyst so she said she would probably put him to sleep there as it’s unfair on him to wake him up at that point. It’s basically just a gamble, whether the cyst will be easily removable or not and the only way of telling will be when he’s in surgery.

I’m basically just asking for advice what to do , or if anyone’s guinea pig has had a cyst (and removed) before?

I’m just absolutely terrified the operation is going to go wrong, either he passes during it or the cyst cannot be removed and they have to put him to sleep. It also scares me that they don’t know what the cyst even is and have never done this operation before so don’t know what to expect, I do trust the vet to do a good job but just depends on what she finds.

I obviously want Gonzalez to be happy and not in pain (if it’s the cyst that is even causing it) but at the moment he seems nearly back to normal (I know he’s on metacam) but its making it so hard to go through with this risky operation as he’s acting sooo happy at the moment, and it breaks my heart to potentially end his life at only 1 year if the operation doesn’t go well. But I also don’t want to risk the cyst getting worst and growing or bursting and causing him pain, but also it might not - might be harmless but we don’t know! It’s so tricky.

I am also sooooo terrified of loosing him as I lost my other 10 month baby guinea to pneumonia in May and I am still processing the guilt and grief of that, I cannot bare to loose both of them and so close together.

If anyone has any advice on cysts or anything or whether I should go through with this operation or not please please do message me, I honestly have no idea what to do.
 
I’m so sorry you lost your little boy in May. And now your other piggy is unwell. I have no experience with cysts but wanted to offer my support nonetheless. I hope you get a good outcome.

I have no experience with cysts either but I wanted to try and ease your worry a little.

I work in a veterinary practice as a nurse and yes you are very correct when you say that it is riskier than operating on say a dog or a cat, but that does not mean the surgery will not be a success. I’ve observed several piggy operations since starting work at my practice and I’m happy to say if anything it has given me more confidence of my piggies ever needed surgery-they are actually little fighters let me tell you we have a star at our clinic who is 8 and I don’t think their is a surgery he hasn’t had!😂 I do advise you discuss your worries with your vet to try and help your nerves. If they felt he wasn’t strong enough they wouldn’t do the surgery at all.

It is unfortunately a tricky scenario here as obviously the surgery is the only option here that has the chance of making your little boy well again. Dosing him with the meds can help but obviously only so much in this particular case.

I send huge hugs to you as I know it is such a scary decision to have to make. Just have faith in your little boy (he sounds like a fighter) and your vets it sounds like they’ll do all they can for him.

My sow Bella recently had surgery to be spayed due to cysts on her ovaries, which is abdominal surgery. I was also very worried that she wouldn’t make it but she did, and has come through the op and recovery really well. I know it’s not the same as with your boar, but I want to tell you about this positive experience to give you courage to go ahead and get your boy sorted. Whatever the outcome, you will have done everything for him and there is no need to feel guilt (though this is the norm for us guinea pig owners when something happens to one of our pigs). Thinking of you and sending hugs at this stressful time.:hug:

Are they sure it's a cyst? Could it be an internal abscess. We had a piggy with an abscess attached to his bowel. Although it couldn't be entirely removed, due to the location, our vet opened the abscess, drained it and sutured it closed. Freddie was then placed a long course of Zithromax. Freddie made a full recovery. We currently have another guinea pig with a large abdominal lump. Hudson is going to have a scan tomorrow and depending on the results of that, he may undergo surgery to either remove it, or at least to investigate what it is and whether it can be safely removed.
 
Glad to hear Freddie made a full recovery! The vet said an abscess is a possibility but they won’t know til they look inside. It’s two circular 1cm diameter masses filled with fluid sitting close together, she said it’s mobile so hoping it’s not attached to anything.

I am very sorry about your issues. Squeaking when peeing or pooing is never a result of a respiratory problem or caused by an antibiotic. It is either an infection, stones or sludge in the urinary tract or something pressing on the on lower gut/pain radiating into the lower gut.

I've never come across internal cysts (apart from ovarian cysts in sows); it is not something you see in guinea pigs. Hence your problem finding information. It is either very much a one-off or it may be an internal abscess? They are not all that common, either, but they do happen. Operative removal would be the best option in either case.

He‘s very drowsy still and not eating yet but getting critical care down him so hopefully soon he’ll feel better!

Have you been given antibiotics, and if yes, what?
 
That sounds like what happened to Freddie. Has he been prescribed antibiotics? We had Freddie on a long course of Zithromax, post op! The abscess has never returned! x

Yes sounds very similar to Freddie’s operation! The meds he has been put on are:
  • Baytril twice a day
  • Carrying on with Metacam twice a day
  • Metoclopramide syrup (anti sickness) four times a day - I think this is to help him feel less sick from op and start eating
  • Critical care every few hours depending on how much he’s eating

They have sent off a swab of the pus they found in the abscess so in a few days should know more about it and what antibiotics will treat it best. I’m intrigued about Zithromax, I will ask the vet about this when she phones me with swab results.

He was very groggy when he got home but started eating a little before the night, and during the night I checked on him every hour and he’d eaten a little veg and was munching on hay and grass.

At 5am when I checked on him , his breathing was very wheezy and loud but it didn’t look like it was fazing him as he was still eating hay and pellets. I was concerned as it was very loud and was every breath, also then some breaths sounded snotty. He went into the vets quickly this morning for them to check lungs etc which they said were fine, just maybe the stressful operation has caused it. His eyes are also bit watery / milky looking in one but again vet said this is fine as probably just from the operation.

Apart from his breathing, he’s doing good, eating a lot more this morning and moving around a bit.

Hi!

Zithromax (azithormycin) is a not licensed strong antibiotic that is nevertheless very effective against especially abscesses in guinea pigs.

My Dylan needed a double course and a month on a high dose of metacam when he developed a jaw abscess just before we were booked for a family Christmas visit abroad. But while he lost his appetite in the last week before his check up in the New Year as soon as the clinic was open again and had to be syringe fed around the clock, much to our surprise the abscess had completely dissolved and has stayed put since then. Zithromax has also saved the eyes of two of my older/frail piggies where an eye removal operation after a bad eye injury was not an option.

Wheezy breathing in the day after a GA can be an after-effect of it and should go away on its own.
