You will need to see a vet as soon as you can.

Please have him see by the vet. That’s not his grease gland, it looks like his bottom bits are pushing outwards somehow. How long has he been like this and how has his pooping been?

i just noticed it today. he’s been completely fine behavior wise. pooping and peeing as normal
 
he has been pooping fine? could it still be impaction?

The guides I linked in explain that impaction doesn’t always stop them passing normal poops. It also explains that impaction can be secondary to other health conditions. It may also not be impaction, but with a swollen bottom in a boar, it is just one of the possibilities
Please do see a vet for a diagnosis

When you have health concerns, then it is best to switch from the routine lifelong weekly weight checks and instead weigh them daily as it can help you gauge what is going on with their hay intake. Hay intake is usually the first thing to reduce if they are feeling under the weather.

Good that you could secure an emergency appointment.
Prolapses from the anus are much, much rarer than impaction but they can sadly happen in both genders.

I am keeping my fingers crossed for your boy.
