My Guinea pig is running around the cage like he has zoomies but it’s more like an uncontrollable spasm. After each one, he drags and bites his genitals and twitches his backside as if he’s in pain or something is causing irritation. I checked and I see nothing. I offered a strawberry, he started to eat but was interrupted by whatever was bothering him again. Pls help asap

There is not a specific set of behaviours here which sound concerning in that something is definitely wrong. What you are describing could all be totally normal behaviours. He could simply have the zoomies and have had an itch. The bottom dragging may well be normal scent marking; they will lick at their genitals; bottom twitching could be dominance rumbling and swaying. He could simply be showing off to his cage mate and having a good time.
However we can only base your written word on an image which it conjures in our minds based upon knowing behaviours in our own piggies.

It can be just his personal brand of zoomies and popcorns.
However, have you also checked the penis shaft for potential semen rods or a major build up of smegma or have you just checked the outside? You need to express the penis for that.
