New Born Pup

This is my first post here. I am at a point where I am not sure what to do.
Last year we found some lumps on my oldest piggy (4.5 years) and took her in for a vet visit. My vet assumed it was ovarian cysts and so we had her spayed. Turns out she did not have cysts, but instead those lumps were her kidneys. She had enlarged kidneys, described to me as balloons. Slowly losing its mass and being filled with water. Based on what my vet said, it sounded like there was nothing that could be done about that. But she was in good health.
Fast forward to a couple of weeks ago, she started peeing a lot of blood. Took her to the vet, she had a UTI. She was put on antibiotics. Well after a week of antibiotics, something strange happened. She suddenly could not move her back legs. She was falling every time she tried to move. Back to the vet, we got an x ray done and a urinalysis. She had fluid buildup all over her body, especially her back legs. The vet took out some of the fluid there. The urinalysis looked good somehow though? No infection and they looked at kidney function. Vet said her kidneys don't look great but they aren't at the failing point.
At the end she said she had no idea where the fluid came from.
Back home, she didn't move for two days but she did eat whatever we put in front of her. She still lost over 180 grams which was very concerning. She is on pain killers and lasix to get more fluid out.
But then she started to recover. She can move around, but not super well. She has a pretty major limp and moves very infrequently in small distances. But she eats fine, she has gained 70ish grams, and seems pretty chipper.
After talking with a friend who runs a rescue, I am worried about that fluid buildup. She said it could come back and affect something worse like her heart or lungs. And then it would be a painful death.
I am torn. She seems ok, but I don't want her to die suddenly in pain (the fluid buildup happened overnight).
She didn't win the genetic lottery. Her sister has already passed from Hodgkin's lymphoma 2 years ago.
Is it time to make the call? What could that fluid be from?
We have another check up in a week. But I am not sure if I will learn anything new. Should I get another x ray done to look for more fluid buildup?
I am sure there is no one right answer here, but I appreciate any advice. Especially if someone went through something similar with kidney problems.
