Blood in urine mystery

Hoping someone may have had a similar experience to me and may be able to shed some light on what might be happening to my poor Guinea Sausage. Sorry for the long story!

A month or so ago I noticed blood in her wee, so I took her to the vet, they did an examination and said she was tender around her bladder. They did an ultrasound and X-ray but saw no evidence of stones, so put her on some antibiotics and anti-inflammatories, followed by painkillers. She has now been on three courses of antibiotics and no improvement. She is also rapidly losing weight and in a lot of pain, will squeak when urinating and when touched around her bladder. I got a second opinion, who also said no sign of stones but did see some thickening on the bladder wall. They also did a urine test and found some bacteria that should be tackled by most antibiotics, so if it was an straightforward infection they would have expected to have seen an improvement by now.

I’m at a bit of a loss as what to do next, she is rapidly losing weight and in a lot of pain. The vet has suggested surgery to go inside the bladder but they are very unlikely to find stones which would be the only cause they would be able to resolve down that route, if they found a growth they would recommend not waking her from surgery.

Has anyone come across a scenario like this before and have any ideas to what the cause could be?

I’m sorry to hear she is unwell. A few questions to get a better picture of what you have done so far

What antibiotics has she been given and the length of each course?
How much pain killer is she on? Dog or cat metacam?
Is she being syringe fed to stop the weight loss?
 
the most recent antibiotics are enrocare (1 week and just been provided more), was on baytril for 1 week and another for 5 days but unfortunately can’t remember which (could find out from previous vet). I’m not sure on which metacam it is (giving 0.35ml if that sheds any light), and for pain was previously on calpol and gabapentin (0.1ml of calpol and 5mg of gabapentin I think it was, didn’t seem to ease much).

The vet hasn’t suggested the syringe feeding but I will start if you think that will help (she has still got an appetite strangely, will come up to me for veg, eating hay, will graze on grass if out outside)
 
I'm pretty sure enrocare is the same thing as baytril. Enrofloxacin is the active ingredient in both.

Syringe feeding when weight is being lost is essential.
Hay is the main part of their diet but because you can’t judge hay intake by eye, their weight is the only way to know they are eating enough. Losing weight means hay intake has dropped so syringe feeding when 50g is lost is essential. You syringe feed as much as is necessary in a 24 hour period to stabilise the weight.
How much weight has she lost?

Is that 0.35ml once or twice a day? Piggies can take higher doses of pain meds and take it twice a day as they metabolise them in about 12 hours. Metacam is also an anti-inflammatory.
Cat metacam is 0.5mg/ml and dog metacam is three times stronger at 1.5mg/ml. Cat metacam is licensed for piggies so tends to be the go to version for most vets. Exotic and knowledgeable vets may still prescribe dog metacam knowing piggies can take higher pain meds.
