Blood clots

Hi,
I'm looking for some help for my Guinea pig Rosie please.
She's had an ongoing problem with a Uti for nearly a year now and things have progressively gotten worse. Just to give a short history up to now
It started with a uti, she was given baytril which did help at first but the uti, or what we thought was a uti came back, as she was still squeaking when she was weeing, she had 3 more courses but it didn't make any difference.
She had a scan back in November, she had some bladder sludge but no stones, the problem persists and in January she has an xray and a scan and they still couldn't t find anything significant. In February she passes a blood clot, after giving her water her wee goes back to normal. The vets culture a water sample and there's no bacteria and no sediment. We start her on cystease and potassium citrate. The potassium citrate didn't have any effect so continued with the cystease. I'm nearly at the end of the third bottle of cystease and it hasn't made any difference at all. Rosie is still squeaking every time she wee's and grunts a little when she comes to the end of the wee. She's been on a 0.2ml dose of metacam for a few weeks now and this doesn't seem to make any difference. Yesterday morning I woke up to blood and bloods in the cage. Rosie still ok, ate her breakfast and her dinner and I gave her water throughout the day but she became worse in the evening, squeaking and hunched over, like she was trying to wee or pass something. I took her to an emergency appointment at the vets and they gave her some pain relief in an injection, which helped and she then had lots of normal wee's without any blood or blood clots which I was really pleased about but was surprised as she hadn't done that all day and I'm wondering if it is IC that she does have as the cystease has had no effect on her at all.
Since having her first uti she's been on a low calcium diet drowned in water from a water filter, we're in Birmingham so have softer water, she has access to lots of hay and a small amount of science selective grain free food. She doesn't drink at all so syringe her water twice a day. I'm taking her back to the vets this morning to see what they suggest. I really don't know where to go from here, I know they will want to repeat an xray or scan but after not finding anything previously, can bladder stone's form that quickly? I'm at my wit's end, any help or advice you can give would be much appreciated.

I don’t personally know how much cytease is needed to help ease symptoms, but if you tell us how much she is having then those who do know may be able to help.

You say she is on 0.2ml of metacam.
Is that dog or cat metacam?
is that once or twice a day?
What is her weight?

Eg 0.2ml of cat metacam, is very low so if she is only having that once a day then I wouldn’t be surprised to hear it isn’t making a difference

It’s good you’ve got her on low calcium pellets and filtering her water but some veggies can make IC worse, so if you could tell us a bit more about her veg diet also

Thank you for your quick reply.
Rose's on 1 capsule of Feliway cystease which is 125mg, I gave her 2 capsules yesterday as her symptoms were worse.
She's on metacam for cats and Guinea pigs which is 0.5mg/ml she has 2 x 2ml yesterday.
Her diet mainly consists of cucumber, green beans, peppers (all colours) the odd blueberry, strawberry and top or a bit apple as a treat(not every day) spring greens and romaine lettuce.
