Specialist Black Skinny Pig -- Babar, ear infection? Neurological issues?

Hi friends,

I'm looking to see if anyone can provide an opinion regarding the situation of one of my pigs -- a black skinny pig by the name of Babar. Five days ago, Babar began tipping over and falling. Initially, I thought this was perhaps attributed to a sprained paw he had experienced a couple weeks back from his cage mate. Fast forward a couple of days and Babar began to profusely trip and fall, struggling to stay up.

Three days ago, I took Babar to the vet to inquire on whether or not his lack of balance was attributed to his sprain. The first vet noted that there was no sprain/skeletal issues with Babar, and that the issue is perhaps neurological (i.e.: meningitis). She then prescribed Babar a 7 day supply of metacam, to be given to him every morning at a dosage of 0.15ML.

During that same night, Babar's balance became significantly worse and upon closer examination of his ears, I noticed what resembled white fungus (see pics provided). Yesterday (Sunday), I took Babar to another vet to receive a second opinion. This vet noted that he could indeed have an ear infection, though his behaviour is more inline with that of a neurological issue. He prescribed Babar ear drops (Surolan) for his ears, to be applied every 12 hours along with Chlor palm (0.5 ml). He also manually cleaned out Babar's ears (to much of his discomfort) and excreted a significant amount of gunk.

Since yesterday, his behaviour has not particularly improved. His balance is still quite off and his pupils are gradually becoming more dilated. Despite the lack of balance, Babar has continued to his vegetables every morning and makes attempts to consume his hay. Drinking water has been a bit difficult for him and I've been providing him with water manually either by holding the bottle directly towards his face and via syringe following Critical Care feeding to which he's been quite receptive.

Is there any advice one can provide regarding Babar's behaviour? If this happens to be an ear infection, how long is the anticipated recovery? Has anyone experienced something like this similar with one of their piggies?

Any insight regarding this would be greatly appreciated!

I haven't dealt with an ear infection before but know they can take longer to clear up. Is the second vet particularly knowledgeable with guinea pigs? I know it's difficult in Canada often to find a good vet and the first one clearly wasn't as they only prescribed pain relief. Surolan is used for fungal infections so it could be worth asking the vet for an antibiotic alongside it incase the infection is not fungal based.

If you only saw the vet 3 days ago it is still early days, it may take time for the treatment to take effect. Surolan is often used as an antifungal skin treatment in piggies, but the information regarding its common on-label use (in dogs and cats) says it is actually also an effective treatment for bacterial ear infections, as well as fungal diseases. So I think this treatment is appropriate :) Only time will tell if your piggy will recover his balance after an ear infection, but if you caught it early and are getting treatment, hopefully in a couple of weeks he will be back to normal x
 
Please give it more time; a course of antibiotic will take at least a week or even several.

Do you also have rabbits where you live?
 
Update: Babar has been much more keen to eat and drink on his own. He's gained 80 grams (despite not particularly losing any weight throughout this whole ordeal) and continues to be reactive to treats and his cage mates (all in separate cages). However, his balance still seems to be quite off and while he's not flopping frantically anywhere near as frequently as he did two days ago, he's still having a hard time keeping balance while remaining static. Should I give it a full week with his antibiotics to see whether or not another vet visit is necessary? Really appreciating all of your support thus far. <3
 
I think given he’s putting on weight and eating, I would give it a few more days for the medication to work.

however, if you’re worried or he seems to be going downhill, or stops pooping and peeing then I’d take him back to the vet.
 
I would give it some time for the antibiotics to work... he may well need a longer course. It's a positive sign that you are seeing some improvement. Fingers crossed he makes a good recovery!
