Appetite not really returning after treatment and meds

Oscar stopped eating properly a couple of weeks ago. He seemed to want to eat and followed us round the house more than normal begging for treats, probably as he was hungry, but once we noticed he wasn't eating them we went to the vet. They said his teeth were fine and everything else was OK in the mouth so gave him critical care and a gut stimulant. The next day I watched him closely and when he did eat selectively (small bits of coriander) he seemed to chew on the side and dribble. I went back to the vet with this extra information and the next day they put him under sedation and found a wound, probably from hay, at the back of the mouth. They removed food debris from it and gave antibiotics and pain killers. That was 8 days ago. I'm still feeding critical care 4 times a day so that he takes in enough food. He will eat small amounts at a time of fresh food, very small amounts compared to normal. Mostly he noses around the bowl and runs off. He is now back to waiting for his morning bowl with his friend at the usual time but barely touches it, just a few leaves. He then goes to Ed's bowl for a look too before wandering off. Critical care is given after I have let him try his normal food as I want to check if he is back to eating by himself first.
Currently I am still feeding him critical care. After this it does seem to stimulate him to eat some minimal food offered too him by hand on my lap cucumber, carrot, herbs, dandelion leaves. I would say hald a 1cm slice of cucumber, but only tne centre of it, then maybe changes to nibble a wedge of carrot and maybe a sprig of parsley if lucky but he usually refusing any more by then. He doesn't seem to be eating leaves like salads or spinach even though he chooses to take those from the bowl. He won't accept random treats away from feeding times. He keeps going to the hay tray and nibbling, but not much, and I haven't seen him touch nuggets. Should he be eating more on his own by now, its almost 2 weeks on critical care?
He is happy in himself and active to a certain extent, in that he would normally be more active in following me in the kitchen for treats. He seems to want food from the bowl but doesn't eat what he takes out and only consumes small amounts. The vet suggests he may still be in pain from something remaining in the wound area (I can't tell if he his, but then he has been on medication and the vet suggested keeping up the pain relief anyway) and she suggests the next step would be a referral for a scan then a surgery, obviously costly so I just wanted any advice before I go down that route. His weight is being maintained so I am not worried about that at the moment. Many thanks for any advice.
 
Hay is the more important thing so his eating bits of veg here and there isn’t really helpful. It may be he’s maintaining the weight through the syringe feeding. Are you weighing him daily or weekly?

I would consider having him scanned to see if there are any other issues with his teeth/mouth or anywhere else. Would the surgery be after the results of the scan?

Is your vet experienced? I see you mention a referral. What was done to the wound in his mouth? Did they think the molars may have caused the wound? And were the teeth checked thoroughly and cleared as okay?
 
Hi, thanks for replying. I do have complete faith in my vets from previous surgeries etc on other piggies. Yes, they checked the teeth on the first visit and then again under anaesthetic when they found the wound. The wound is more at the back I believe near the throat and not caused by teeth. Months ago my husband phoned just after I left for work as he saw Oscar struggling with hay stuck in his throat but when I rushed home (literally 2 mins) he was ok already. Now this was far too long ago to be causing recent issues but,we wonder if he caught the spot again? The wound was cleaned up but I don't have specifics, it didn't require stitches tho.
The vet is an exotic but they don't have the relevant scanner so the referral was to a specialist that does I believe.
Oh and while holding him I have noticed a teeth grinding type noise on occasions. I don't know if this is new really as until this illness Oscar hasn't liked cuddles, far too active, but he has been cuddling for long periods now.
I realise he needs the hay and he does pop in there quite a bit, I just can't monitor how much he's digesting really.
 
It’s good you trust your vet. Weighing is the way to monitor how much hay he’s eating, which is what you’re doing. I’d ask for the referral. I don’t think teeth grinding and the change in behaviour should be left.

Hope you get some answers soon and he picks up.
